Services for Seniors
The State of Minnesota offers two programs for people age 65 and older to get support and services at home: the Alternative Care (AC) Program and the Elderly Waiver (EW).
People who receive these programs get help from a case manager and can choose services that best meet their needs, based on an assessment. Eligibility for these programs is determined through the MnCHOICES Assessment.
What is a waiver program?
A waiver program is a type of program provided and allowed by State of Minnesota to help people with disabilities, seniors, or anyone with specific medical needs get the care and support they need to live independently and remain in their homes, rather than moving to a nursing home or other care facility.
Available waiver programs
- Alternative Care (AC) Program (DHS website)
The Alternative Care (AC) Program is for people over age 65 with low income and assets who do not qualify for Medical Assistance. It provides financial help for seniors, who need the level of care offered in a nursing facility, to receive home care services and supports instead. The program doesn't include Assisted Living services. - Elderly Waiver (EW) Program (DHS website)
The Elderly Waiver (EW) helps people over age 65 who are on Medical Assistance. This program provides services and supports to help them live independently and stay in their home and community instead of moving into a nursing home. The services offered are tailored to each person's needs and choices.

- Consumer Directed Community Supports.
- Adult day care programs.
- Caregiver support.
- Case management.
- Home health aide and nurse visits.
- Homemaker.
- Supplies and equipment.
- Short-term respite.
When a person is receiving waiver services
- When someone is receiving waiver services, their service plan is managed by a social worker or nurse at Washington County. This professional authorizes and monitors the services the person is receiving, making adjustments to the plan as needed.
- The plan is reviewed annually to ensure the person still meets the eligibility requirements and that the services are meeting their health and safety needs.
