Notice: The Headwaters Service Center in Forest Lake is in the same building location as the Forest Lake License Center and the Hardwood Creek Library.

Appointments

• Services from Community Services will continue to be done via phone call or video conference. When required, in-person meetings are by appointment only. To schedule an appointment, call your worker or call 651-430-6455.
• For Public Health & Environment services and appointments, call 651-430-6655. These services include vaccines for income-eligible residents.
• For  Women, Infants, and Children (WIC) appointments, questions and hours, call 651-430-6658.
