The county provides short-term planning and ongoing support to make sure people have the tools to live independently and get the help they need. Our goal is to help people and their families access the resources that can improve their lives.

How to get started: Get a MnCHOICES Assessment

To schedule your assessment, submit the online referral form or call 651-430-6422 (select option 1).

Next, a social worker will contact you and ask you questions to provide information about potential options depending on your age and needs.

Services for children and families with disabilities

If you're a parent or guardian of a child with a disability, the MnCHOICES Assessment can help you access the right services for your child, such as therapy, educational support, and family resources.
