Home and community-based waivers

The State of Minnesota offers disabled individuals, under the age of 65, the following waiver programs to receive in home supports and services. Eligibility for all waivers and services is determined using the MnCHOICES Assessment. See how to request your MnCHOICES Assessment.

What is a waiver program?

A waiver program is a type of program provided and allowed by State of Minnesota to help people with disabilities, seniors, or anyone with specific medical needs get the care and support they need to live independently and remain in their homes, rather than moving to a nursing home or other care facility.

Available waiver programs

Note: All of these waiver programs require the individual to be receiving Medical Assistance and to be certified disabled through Social Security or by a State Medical Review Team.

Services that may be available

When a person is receiving waiver services

  • When someone is receiving waiver services, their service plan is managed by a social worker or nurse at Washington County. This professional authorizes and monitors the services the person is receiving, making adjustments to the plan as needed.
  • The plan is reviewed annually to ensure the person still meets the eligibility requirements and that the services are meeting their health and safety needs.
