For most benefits, you will need to provide military personnel records. You can receive free copies of your discharge document and other military and medical records by:

• Completing an online form from eVetRecs.
• Completing Form SF-180 and mailing or faxing the request.
• Contacting the Minnesota Department of Veterans Affairs.
